SPTE Request Forms

There are two different ways to submit your SPTE Request Form.  First option is the electronic version.  Click on the appropriate semesters link.  The second version is for you to be able to download and print the form. 

* please note that the electronic form will not be available after the due date and you will have to submit a paper version.

Fall SPTE Electronic form - not available for Fall 2012

Spring SPTE Electronic form - DUE DATE February 27, 2013

Summer SPTE Electronic form - 1st 4 week DUE DATE June 19, 2013

Summer SPTE Electronic form - 8 and 2nd 4 week DUE DATE July 15, 2013 

2.  Download paper "SPTE Request Form"  (You may print and mail form to Box 107 or hand deliver it to room 429LH.)
