Please read the documents listed below by clicking on the title of each document. We have included forms that you may need to access during the semester and also procedures you can follow that will assist you and ODS in the process of providing testing services for students with disabilities. During the first two weeks of class a student may bring you a letter and a form from our office requesting that you provide testing accommodations. DO NOT provide Testing Services to any student until you receive the appropriate forms from our office. If a student requests Testing Services and they have not presented the appropriate forms from our office, please have them contact Grady Landrum, Director of Disability Services at 978-3309.
