**Scholarship Application**

Starting Fall 2009, all School of Music Scholarship Applications must be submitted electronically.

Unfortunately, we cannot accept printed or handwritten documents – your information can only be added to the scholarship database via electronic submission.

Application Instructions

1. The Scholarship Application MUST be completed in Adobe Acrobat Reader. This is a free program that may be downloaded from the "Helpful Links" on the right. Do not complete this form in Preview, or you will be unable to submit it.

** Again - This form does not function properly in non-Adobe PDF programs, like Preview**

2. Please completely fill out the application. Select “Highlight Fields” in Acrobat to see all the field areas. The areas surrounded in red are required fields – Adobe will not allow you to submit the form without completing these fields.

3. When you Click “Submit”, you will be given two options:

a. Desktop Email Application – use this if you run an email application such as Outlook, Entourage, Mac Mail, or Eudora. The application will send directly from the “Submit” button.
b. Internet Email – Use this option if you use an internet email application, such as Hotmail, Yahoo, etc. If you are on a public computer, this is the option you will select. You will have to save the file to your desktop, and then attach it in an email:

i. When you click this option, a popup window will offer to save this application. On the “where” dropdown, be sure you save to the Desktop (so the file will be easier to find).

ii. Log in to your email account online, and send this application in an email, as an attachment, to musicscholarships@wichita.edu.

iii. You will receive an automated response that your application has been received.

iv. If you are on a public computer, don’t forget to delete the application data from the desktop.

c. If you are having trouble submitting your application, call Ali Levine at (316) 978-3500; or Jan Ives at (316) 978-6430

**If your form has been submitted correctly, you will receive a confirmation email from musicscholarships@wichita.edu**

4. If you have any questions, please see your Area Director. Please do not send questions to the aforementioned email – this email is for the receipt of scholarship applications only, and questions will not be answered or forwarded.
