State of Kansas Benefits for Wichita State University employees are administered in the Human Resources Center, located between the Rhatigan Student Center and the Campus Activities Center Theater. The office is open 8:00 am - 5:00 pm, Monday through Friday.

| Health Coaching Service | Talk to a personal, specially-trained professional health coach; available 24/7 regarding education and support; questions to discuss with your doctor; educational materials at no cost; to manage ongoing medical conditions. Available to benefited employees and their covered dependents. |

| Insurance, Dental | Delta Dental is the state provider for dental insurance. |

| Insurance, Student Employee | Student employees of the University are not covered under the State health plan. Go to the Graduate School Office or Student Health to obtain application brochures. |

| Kansas Board of Regents Mandatory Retirement Plan | This Investment Agreement is entered into between the undersigned employee of WSU in order for salary deferral contributions to be made to the Kansas Board of Regents Mandatory Retirement Plan under Internal Revenue Code Section 403(b) and K.S.A. 74-4925. |

| Kansas Mentors Program | As a State of Kansas employee you have the chance to help close our "mentoring gap." Each pay period, you are eligible for up to 90 minutes of paid time to volunteer in an approved Kansas Mentors' program. |

| Life Insurance -- Supplemental (Employee and/or Dependents) | This is the optional life insurance coverage available when an employee wants coverage for himself/herself and/or dependents. Contact the HR office for an enrollment form. |
| Long Term Disability | This coverage is provided by the University. Contact the HR office for information if you need to file a claim or have questions. |
| Payroll Checks -- Distribution Schedule | State payroll checks are distributed every other Friday. When payroll falls on a holiday, checks are distributed the day before the holiday begins. |

The Tuition Assistance Program for Spouses and Dependent Children (Effective Date: December 1, 2011) is available to the spouse and/or dependents of all full-time employees at Wichita State University. |
