The application packet consists of the following:

  1. Dental Hygiene Application Form
  2. Course Summary Form
  3. Application questionaire
  4. Dental/Healthcare Work Experience
  5. Dental Hygiene Observation Form (must observe a licensed Registered Dental Hygienist for 1/2 day)
