W-9S Request for Student's or Borrower's Taxpayer Identification Number and Certification

To provide WSU with student’s social security number and receive a 1098-T, students (who have not already provided this information) must complete, sign, and return this form to the WSU Accounts Receivable Office (in Jardine Hall) or mail to: Accounts Receivable, 1845 Fairmount, Campus Box 38, Wichita, KS 67260-0038.
