The purpose for this procedure is to help identify gaps in services, procedures, and policies of the Office of Disability Services (ODS). The goals for implementing this procedure are twofold: 1) encourage communication with students and 2) use feedback from students to improve or maintain services. Students who believe they have received inappropriate, inadequate or unacceptable services from ODS have the right and are encouraged to file an incident report with the Director of ODS. Students are encouraged to recommend options to help rectify the issue; this will assist ODS staff in resolving the incident.
Incident forms are available in the main office of ODS for easy access. Students must take the initiative to file a form on their own behalf. Writing assistance will be provided by ODS if requested. The Incident forms that are filed will be kept confidential. The completed form should be placed in an envelope marked "Confidential". The form may be mailed to Director, Office of Disability Services, Wichita State University, 1845 N. Fairmount, Wichita, KS 67260-0132.
Students can request a meeting with the ODS Director upon filing a report.
Students will receive a written response to the incident report within five school days. Issues will be resolved in a timely manner.
Students are encouraged to file a report within five school days of the incident. This will help resolve issues while the event is fresh in people's memories.
If students are not satisfied with the response from ODS, students may file a complaint with the WSU, ADA/504 Compliance Officer, Gina Crabtree, WSU, 1845 Fairmount, Wichita, KS 67260-0008 or by 316-978-3023. The ADA/504 Compliance Officer will investigate the complaint and reply to the student who filed the complaint within 15 working days.

Is responsible for the monitoring of all construction projects whether they are new building or remodeling of existing buildings or areas within a building including labs, classrooms, and offices or departments.
Provides support and is a resource person for the Campus Access Committee, it’s subcommittees and the ADA/504 Compliance Officer.
Serves on the review panel for all construction plans at WSU.
Chairs the Access Committee in the absence of the ADA/504 Compliance officer.
Works in a consultant role with Human Resources for employees of the university to determine appropriate workplace accommodations.
Is responsible to investigate any complaints filed on campus that address issues concerning people with disabilities, the accessibility of facilities or regarding program access

Consults with University Counsel to resolve ADA/504 Complaints.
Chairs the committee that reviews, responds, and works for resolution of ADA/504 complaint
Any complaint not resolved at the ODS will be forwarded to the ADA/504 Compliance Officer. In consultation with the WSU Legal Counsel an investigation will occur to resolve the incident within 15 days and a written response will be sent to the person who filed the complaint. At the discretion of the compliance officer a committee may be appointed to help investigate and resolve the issue. If a resolution cannot be obtained between WSU and the complainant then the complainant may explore other avenues to find resolution to their complaint.
