Americans with Disabilities (ADA) This Employee Request for Accommodation form is used for reporting of disability status and request for special accommodations.

KBOR Immediate Participation Form This form allows eligible unclassified professional and faculty employees to waive their 1-year waiting period and begin contributing to the KBOR Mandatory Retirement Plan immediately.  Employees must submit this to past employer(s) and have the form returned to Human Resources within 90 days of their hire date to be valid. K-4 State Tax Withholding Form Employees must complete the Form K-4 for the State of Kansas withholding tax purposes.  The withholding elections made on the Form K-4 may differ from the Federal withholding elections made on Form W-4.  *PDF
Kansas Mentors Program Employees may use up to 90 minutes of work time per pay period to work with an approved mentoring program. This time must fall within the employee's regular work schedule and shall not be used during or result in overtime for non-exempt employees.  >>See more about the program.

Medical Leave Request Form This form can be used to apply for a leave of absence including FMLA, workers' compensation, and ADA.  It must be completed & signed by the employee. 
*To be eligible for the Family & Medical Leave benefit, employees must have 12 months of University/State service and have worked 1,250 hours in the immediate previous 12 months.
